CCCC has three membership categories:

  • Organization – $250 per year
  • Community Coalition – $100 per year
    This category is for community-based coalitions working to improve end-of-life care at the local level.
  • Individual – $50 per year

Get Involved!

CCCC has workgroups that focus on specific topics related to end-of-life care and provide input on CCCC’s work. Participation in the workgroups is open to all interested members. Workgroups meet three times a year.

Current workgroups include:

  • Long-Term Care
  • Public Engagement
  • Developmental Disabilities
  • Public Policy
